Actually, 4/4 TED defect does not sound to fit to the "normal" defect pareto we know.

PLA, 8501 and even the memories tend to fail.
Especially C16, but also the Plus/4 has the PSU troubles too. nothing special, same as the C64, but in C16 the zeners were really too weak.

Are you confident, that there was no gaming involved with fancy autofire joytisck (TED current overload) before the defect?
Or they arrived with defective TED?
Because this is a fact, that the TED has no protection on the joystick port.

In that case you really need the TED protector for TLC, otherwise after a TED change the same story will happen.
Also Luca mentioned those adapters, that use GND and not the select line (if I remember well), these also do not help on the TED lifespan. The reomval and connection of joysticks also a potential risk.
(in fact, the mini DIN connector outer shielding better lead the current, than the C64's DB9 plastic housing.)
