I think Acrid is looking for the tape of the original one. The cover was sent in by Sidius in 2005, back then we weren't really collecting the tape photos. Unfortunately he hasn't been around for a long time, though I'd assume several other collectors own a copy of the game, so they should be able to take a photo.
