This is the Ultimate Map (previously Ultimate ROM Map) from SVS. It includes the entire O.S. ROM of Plus4 (C=16), a large TED Registers list, graphic plans of all allowed text/graphic modes, and a table of Plus/4 bugs.
It's in XLS format (you need Microsoft Excel, or another software capable to view XLS files to view it), in order to allow easy searches and sorting columns' data. Download it and open the file for more information.
