Error 21 is a sync mark error, doesn't the 15x1 series read each sector as it's formatted to make sure? This could account for the formatting problem error report (bad disk).
Did you try cleaning the r/w head by gently swabbing with 90% or higher alcohol? I notice you had problems with 1541 reading disks from the 1551 as of December 24. If the 1541 is having troubles of its own, a bad disk (very bad disk) could have either dirtied or damaged both drives' heads if it was dirty or moldy and distorted.
I agree it could be the speed of rotation, if the 1551 is direct drive that's bad, but maybe adjustable. Another possibility, perhaps the disk isn't getting clamped properly. Bad alignment is rarer than other problems, most of which are easier to fix than alignment. If cleaning heads doesn't work, maybe the rails need lubed with molybdenum base lubricant. If the "easy fixes" don't work, you should probably replace the drive mechanism with a known good 1541 mech.
