I tried the XPMConvert utility, and it does indeed reduce the palette to 18 colors, and I did not find a way to disable this feature. The file I posted above was created with the GIMP; loading an indexed mode .bmp file with it and then simply saving it as .xpm seems to preserve the original palette, although I also used the GIMP to convert the image to the Plus/4 palette. There are probably other utilities that would work, too.
I could also build the converter with a modified version of the FLTK library that does not "optimize" the palette of .gif files (which is the reason why the only format that currently works for pixel exact image reading is .xpm).
