Perhaps because the file only contains 128 colors, rather than 256 ? I have replaced plus4palette.zip with a modified version that writes a 256 color (768 bytes) palette if binary format is selected.
However, I am not sure if different palettes are the reason why you have problems with converting .xpm files (the converter should accept palettes that are only similar to trigger the pixel exact reading mode, as long as all colors are at the correct index, for example 113 is white etc.); could you post an example that does not work as expected ?
