The input file should be in .xpm format, and have a palette that looks similar to the Plus/4 palette, with at least 128 colors and no transparency. Here is a simple example file: http://www.sharemation.com/IstvanV/flitest/test.xpm
Try converting it in high resolution non-interlaced FLI mode; notice that the result is not accurate if "optimize for PAL color filtering" is enabled, so it should be turned off for this type of conversion (older versions were not correct in either mode).
The no transparency limitation could be eliminated (in fact, I have intended to that already, just forgot about it), but the restriction to .xpm format is because the library I use for reading images only preserves the colormap of this format, all others are either converted to RGB or unused colors are removed.
