Does the version that was included with plus4emu 1.2.5.2 also crash on the same machine ? If not, then make sure that the file mingwm10.dll is also extracted to the same directory as p4fliconv_gui.exe; the simplest way to get a working installation is to extract the package to a plus4emu installation directory, overwriting any old files (this also allows running the program from the start menu). You can also try the slower non-SSE build in case the problem is caused by the use of special CPU instructions (the previous versions were all compiled with the settings of the "compatibility" package).
Some more questions: does the crash occur immediately on start-up, or the program starts, but crashes later, for example when opening a file ? Does command line conversion using the console version (p4fliconv.exe) work ? What is the actual error message ?
