OK, I have changed the SVN sources so that it is now non-interactive on OS X, using the newly added -f flag.
It is possible to change the keyboard layout later by loading one of the configuration files created by makecfg.
The reason why the ROM setup is uninitialized if the files are installed only after running the emulator is that files that are not found are automatically cleared from the configuration so that errors will not keep occuring repeatedly. So, the ROMs should either be installed before launching the .app for the first time (as suggested by the README), or the configuration needs to be reinitialized by loading one of the presets, or deleting ~/Library/Application Support/plus4emu/plus4cfg.dat and running the emulator again.
While packaging the ROM images with the releases is technically easy to do, I am not sure about the possible licensing issues, since it is only allowed to host open source free software on SourceForge.
